Voluntary Pre-Kindergarten (VPK)
VPK Eligibility Documentation
To be eligible for VPK, you must provide the following documentation:
1 Proof of the Child's Age and
1 Proof of Florida Residency
Please also bring your child's Social Security number.For Proof of the Child's Age:
birth certificate
physician signed shot record
military ID
passport
For Proof Of Florida Residency:
The Proof Of Residency Must Match The Address Where The Child Resides, Which Should Be Reflected On The Application
current Flordia driver's license
current Florida state ID
mortgage statement (showing address and signature) lease agreement (showing address and signature)
recent utility bill (electricity, water or cable)
property tax assessment
vehicle registration
pay check stub
tax return
unemployment letter
Effective August 31, 2009: Bank Statements, Homeowners Insurance And Vehicle Insurance Will No Longer Be Accepted As Proof Of Residency.
Do I need copies? We will be happy to make copies of your documentation for you when you enroll. However, you are welcome to make your copies ahead of time, if you choose. It may make the process faster.
